Best Doctors Centers of Excellence: What It Really Is and How It Works in 2026

If you work with international health insurance clients, you have surely heard about the Centers of Excellence benefit from Best Doctors Insurance (BDIL). You have probably also heard incomplete explanations: “it’s access to Mayo Clinic”, “it covers Cleveland Clinic”, “it’s premium treatment at no cost”. None of them is accurate. This article explains, based on the official terms and conditions published by BDIL on May 11, 2026, what the benefit really is, how it works, what it covers, what it excludes and how to make the most of it without creating false expectations for the client.

In one sentence: Centers of Excellence is an incentive that waives the deductible for the first eligible hospitalization or surgery of the policy year, provided the insured is treated at a center on the official list and coordinates the event with Best Doctors Patient Services under the direct payment modality (not reimbursement).

What Centers of Excellence really is

The Best Doctors Insurance Regional Centers of Excellence program is not a premium hospital network with automatic access. It is a well-defined financial incentive: when the insured needs their first eligible hospitalization or surgery of the policy year and chooses a provider from the Centers of Excellence list, Best Doctors waives the deductible applicable to that event.

The benefit took effect on March 1, 2026 and applies to all in-force and new policies of the Pro Suite products (with some exceptions we will see below), regardless of the insured’s country of residence. The list of Regional Centers of Excellence includes high-complexity medical centers selected by BDIL in different regions.

It is important to understand what it is not : it is not a free-access card to any premium hospital in the United States, it does not cover diagnostic tests or outpatient therapies, and it does not apply to reimbursement. It is a real and valuable benefit, but a limited one.

How the benefit works, step by step

For Centers of Excellence to apply, the sequence must be followed in the right order. If the insured skips a step, they lose the benefit.

  1. Prior contact with Best Doctors Patient Services. Before scheduling the hospitalization or surgery, the insured (or their treating physician) must contact BDIL’s medical teams to verify eligibility of the event and the provider.
  2. Coordination with the medical center on the list. Best Doctors helps coordinate care with the selected hospital, ensuring the direct payment modality is followed.
  3. Treatment under direct payment. BDIL pays the provider directly for hospital expenses, supplies and medical fees according to UCR (usual, customary and reasonable). The insured does not advance any money.
  4. Deductible waiver applied to that event. The deductible the insured would normally have paid is waived for that hospitalization or surgery.

A later reimbursement attempt without prior coordination does not qualify for the benefit. This is the most common operational mistake.

What exactly it covers

The benefit covers the first episode of care of the policy year for each insured, at a center on the list, including:

  • Inpatient hospitalization
  • Elective and scheduled surgeries
  • Outpatient surgeries (no overnight stay)
  • Hospital expenses associated with the event
  • Supplies and instruments used
  • Medical fees according to UCR

What it does NOT cover (even within the same episode)

These exclusions are explicit in the official terms and apply even if they are directly related to the main hospitalization or surgery:

  • Outpatient emergencies
  • Subsequent readmissions
  • Physical therapy and rehabilitation
  • Minor surgical or diagnostic procedures: sutures, placement of chemotherapy ports, endoscopies, colonoscopies, biopsies
  • Diagnostic imaging: CT scans, MRI, ultrasounds

The basic rule: if the plan has no coverage or very limited coverage in the United States, it does not qualify for the program.

Critical rules that prevent surprises

These are the rules that generate the most misunderstandings with the insured. Explain them to the client before selling the benefit as a differentiator.
  • Only once per policy year and per insured. If the first eligible hospitalization has already been used, the second one pays the normal deductible again.
  • Direct payment only, no reimbursement. If the insured pays first and submits an invoice, the benefit does not apply.
  • Requires prior coordination. Without advance contact with Patient Services or BD’s medical teams, the benefit is not activated.
  • Non-transferable. If it is not used in the policy year, it is lost. It does not carry over to the next year.
  • The remaining deductible stays in effect. Once the waiver is applied, the policy’s remaining deductible applies to all future medical services of the year.
  • Not combinable with other benefits: it does not apply together with Your Best Access, carryover, waiver for serious medical accident, waiver while traveling, or vanishing deductible.
  • Best Doctors reserves the right to cancel or modify the program without prior notice.

How to make the most of it strategically

Properly understood, Centers of Excellence is a real savings lever for the insured. These are the three most useful plays:

1. Plan elective procedures. If the insured has a scheduled surgery that can be timed (orthopedics, gastroenterology, elective cardiology), scheduling it as the first intervention of the policy year maximizes the savings.

2. Always coordinate in advance. Before setting a date with the specialist, the insured should call BD Patient Services to validate that the chosen center is on the Regional Centers of Excellence list and that the event qualifies.

3. Do not use the benefit for small events. If the first intervention of the year is minor (low deductible, limited amount), it is better to save the benefit for a larger intervention expected later in the year. This is where the advisor can add real value by guiding the decision.

Frequently asked questions

Who can use the benefit?

It applies to all issuers, in-force policies and new business of Pro Suite products, with the exception of Medical Select Pro Internacional and Brasil. Regardless of the insured’s country of residence.

Is it useful if I am treated in Argentina, Colombia or Mexico?

Yes, if the provider is on the Regional Centers of Excellence list. Best Doctors publishes the updated list and Patient Services confirms eligibility case by case.

Does it work with reimbursements?

No. The benefit applies exclusively under direct payment with prior coordination. A later reimbursement without advance contact does not activate the waiver.

What happens if I am readmitted after surgery?

The readmission no longer qualifies for the benefit. The waiver covers only the first episode of care.

Can I combine it with other benefits of my policy?

No. During the first hospitalization or surgery covered by Centers of Excellence you cannot combine it with carryover, waiver for serious medical accident, waiver while traveling or vanishing deductible. Those benefits remain available for later events in the year.

How many times a year can I use it?

Only once per policy year and per insured. Each covered member of a family policy has their own independent benefit.

If I do not use it during the year, does it carry over to the next?

No. The benefit is non-transferable. If it is not used during the current policy year, it is lost.

Can Best Doctors change the conditions?

Yes. The company reserves the right to cancel, modify or revise the program without prior notice. This article reflects the official terms in force as of May 11, 2026.
